      Client can follow step by step instructions on the web page. Client will need their online confirmation number when contacting state regarding their application. After completing online registration, client can call the state in about 3 business days to obtain the account number and rate information.

      Information needed to complete AID Registration:

      You will need the same information required to complete the paper application (Form DR-1):
      Full name, social security number, home address and telephone number, and driver license number of the business owner, or a corporate officer

      Federal Employer Identification Number (FEIN), if applicable, for the business entity; if you are registering for Unemployment Tax, you must have an FEIN. If you do not have an FEIN, visit the IRS web site to apply for and receive your number online

      Physical and mailing address of the business entity

      If incorporated, date of incorporation, fiscal year ending date, and Florida corporate document/registration number (assigned by Secretary of State, Division of Corporations)

      If you rent your business location, and do not operate from your home, the name, address and telephone number of the property owner or manager

      Signature Required: When completing the application interview, the business owner or corporate officer must electronically sign and attest to the application's accuracy and completeness. This application becomes a legal, binding agreement once submitted.
      Confirmation: Once your application is successfully submitted, you will be presented with a printable document containing all of your responses as well as your confirmation number. We recommended that you print and save the confirmation page until you receive your registration documents by mail.
